Special Events and Farmers' Market Notification/Application Forms
Food served or sold at community festivals, fundraising events and farmers' markets are an important part of local life in communities across Ontario. Before planning fundraisers, organizing a special event or a farmers' market where food will be sold or provided to large numbers of people, take the time to do some careful food safety planning. Food safety is everyone’s responsibility.
Notification and applications forms must be made at least 15 days prior to the event. Your local public health inspector will assess your application and explain the health regulations that are in place to help prevent foodborne illnesses caused by improper food handling, storage and preparation. We look forward to working with you toward a safe and successful event.
Special Events
For special events, please complete the Special Event Application/Notification form and forward to your local Porcupine Health Unit Office.
Farmers’ Markets
For farmers' markets, please complete the application for Farmers’ Market Organizers and have all vendors at the farmers’ market complete the application for Farmers’ Market Food Vendors. Forward completed applications to your local Porcupine Health Unit Office.
