Exemptions for Immunizations for Children Attending School or Daycare

If you choose not to immunize your child for medical, religious, or philosophical reasons you must submit either a Statement of Medical Exemption or a Statement of Conscience or Religious Belief.

Statement of Medical Exemption

The following steps must be completed to ensure your child is exempt from the vaccination for medical reasons:

  1. Complete the Vaccine Exemption Form.
  2. Complete the Statement of Medical Exemption Form (school) or the Statement of Medical Exemption Form (daycare). Speak with your physician to have this form completed. This form must be signed by a physician or nurse practitioner.
  3. Return the original signed copy of the Statement of Medical Exemption Form along with the Vaccine Exemption Form to the Porcupine Health Unit.

Statement of Conscience or Religious Belief

The following steps must be completed to ensure your child is exempt from the vaccination for conscience or religious beliefs:

  1. Complete the Vaccine Exemption Form.
  2. Complete only the top section of the Statement of Conscience or Religious Belief Form (school) or the Statement of Conscience or Religious Belief Form (daycare).
  3. Have the form signed and stamped by a Commissioner of Oaths, a Notary Public, or a Justice of the Peace.
    1. Obtain a Commissioner of Oaths from your local municipal or MP/MPP office. Contact the office beforehand as some offices require appointments. Ensure to ask if you need to bring one or two forms of identification. Fees may apply.
    2. Contact a legal office to have a Notary Public or a Justice of the Peace to sign and stamp the form.
  4. Return the original signed copy of the Statement of Conscience or Religious Belief Form along with the Vaccine Exemption form to the Porcupine Health Unit.
