Exemptions for Immunizations

Exemptions - School

Ontario’s Immunization of School Pupils Act (ISPA) requires that parents of children attending primary and secondary school provide their local medical officer of health with proof of their child’s immunization against designated diseases. Children of whom the public health unit does not have required immunization records or a valid exemption may be temporarily suspended from school until the required records are provided.

Vaccines currently required for school attendance include: Diphtheria, Tetanus, Polio, Measles, Mumps, Rubella, Meningococcal disease, Pertussis (whooping cough), and Varicella (chickenpox).

Parents wishing to submit an exemption will need to contact one of our nurses to review the new process. As of September 1, 2017, all parents requesting a conscience or religious belief exemption to vaccination for children attending school will need to participate in an education session offered by the health unit in addition to submitting their affidavit.

Statement of Conscience or Religious Belief Affidavit

The following steps must be completed to ensure your child is exempt from the vaccination for conscience or religious beliefs:

  1. Complete the one on one education session at the Porcupine Health unit;
  2. Complete the Statement of Conscience or Religious Belief Affidavit:
    1. the affidavit must be signed and stamped by a Commissioner of Oath (which can include a Notary Public, or a Justice of the Peace). Your local municipal or MP/MPP office can help you locate others.
    2. Fees may apply.
  1. Return the original signed copy of the Statement of Conscience or Religious Belief Affidavit.
  2. Once the Affidavit is received and the education session is complete, then the exemption will be processed.

Statement of Medical Exemption

The following steps must be completed to ensure your child is exempt from the vaccination for medical reasons:

  1. Complete the PHU Vaccine Exemption Form.
  2. Complete the Statement of Medical Exemption Form
  3. Speak with your physician to have this form completed. This form must be signed by a physician or nurse practitioner.
  4. Return the original signed copy of the Statement of Medical Exemption Form along with the PHU Vaccine Exemption Form to the Porcupine Health Unit.

Exemptions for Immunizations for Children Attending Child Care Centres

Ontario’s Child Care and Early Years Act (CCEYA) states that a child care centre must ensure that all children in their centre have complete immunization appropriate to their age prior to admission to the child care centre. A record of immunization must be kept as part of each child’s record.

Vaccines currently required for child care centre attendance include: Diphtheria, Tetanus, Polio, Measles, Mumps, Rubella, Meningococcal disease, Pertussis (whooping cough), and Varicella (chickenpox).

Statement of Conscience or Religious Belief Affidavit

The following steps must be completed to ensure your child is exempt from the vaccination for conscience or religious beliefs:

  1. Complete the PHU Vaccine Exemption Form
  2. Complete the Statement of Conscience or Religious Belief Affidavit (for child care centres):
