Immunization Records (ICON)

Access and print an Immunization Record (Yellow Card)

You can use ICON (Immunization Connect Ontario) to view or print an immunization record for you or your child. ICON is a new provincial program that allows the public to access their immunization records and obtain yellow cards. The site is maintained by the Ontario Ministry of Health and Long-Term Care, not the PHU.

This site should NOT be used to record your COVID-19 vaccination. If you upload your COVID-19 vaccine record, it will be rejected. All COVID-19 vaccines are recorded on COVaxON and a copy of your record can be accessed on the Ontario Health website.

What you’ll need

In order to access ICON, you’ll need your (or your child's) Ontario Health Card. The site will prompt you to create a PIN. If you run into any problems or have questions, please call the Porcupine Health Unit at 1-800-461-1818.

Please note:

  • Do NOT record your COVID-19 vaccinations on this site.
  • This website is for people living in the Porcupine Health Unit region.
  • Contact your local health unit for your immunization record.

Staying up-to-date

If you are looking for your immunization records, the health unit may have a copy.  However, these records are often incomplete, depending on where and when you were vaccinated.

If you or your child got their immunizations from either the PHU or a health care professional in our area, the vaccinations will likely already be recorded in ICON.

However, you can use ICON to submit recent immunizations to us. If you’ve received a notice from us that your child requires immunizations, you can use this system.
